Ozzy Osbourne's official cause of death has been revealed as a heart attack, according to his death certificate. Coronary artery disease and Parkinson’s disease were listed as "joint causes.” Osbourne died on July 22 at the age of 76 just weeks after reuniting with his former Black Sabbath bandmates.
